37.106.1432 PERSONNEL FILE REQUIREMENTS (1) The administrator or designee must ensure there is a current secured personnel file for each employee and trainee and for each student, volunteer, and contract staff person who provides or supervises client care. The file must include: (a) the results of a tuberculin test upon employment and annually thereafter; (b) a criminal justice information network (CJIN) background information check on each staff person having direct contact with clients; (c) evidence that all staff who provide or supervise client care have current and valid certification in cardio-pulmonary resuscitation (CPR) and in first aid techniques; (d) an annual performance review that is: (i) conducted by the appropriate supervisor of each staff member; and (ii) signed and dated by the employee and supervisor. (e) copies of registration or licensure applicable to employee's job duties; (f) evidence of an independent contractor status and contractual agreements for contracted personal; (g) a signed statement acknowledging the employee has been oriented and agrees to abide by confidentiality requirement to maintain confidentiality of client information; (h) resume or job application; (i) disciplinary actions and grievances; and (j) a copy of a current job description, signed and dated by the employee and supervisor which includes: (i) job title; (ii) minimum qualifications for the position; and (iii) summary of duties and responsibilities.
