6.6.606 PLAN CHANGES TO BE FILED AND APPROVED (1) A medicare select issuer shall file for approval with the commissioner any proposed changes to the plan of operation, except for changes to the list of network providers, prior to implementing such changes. (2) An updated list of network providers shall be filed with the commissioner at least quarterly.
