(1) The following forms shall be utilized by the department and may be obtained from the Transportation Division of the commission, 1701 Prospect Avenue, Helena, Montana 59620-2601.

(a) Form 1. Class A and B motor carrier merchandise or commodity (cargo) liability certificate of insurance, stock form no. 126.

(b) Form 2. Common and contract motor carrier automobile bodily injury liability and property damage liability certificate of insurance, stock form no. 125.

(c) Form 5. Uniform notice of cancellation of motor carrier insurance policies, stock form K.

(d) Form 6. Uniform motor carrier cargo certificate of insurance, stock form H.

(e) Form 7. Uniform motor carrier bodily injury and property damage liability certificate of insurance, stock form E.
