(1) A licensed monitoring well constructor shall prepare a monitoring well report form for each monitoring well drilled. The monitoring well constructor shall supply copies of the report to the monitoring well owner and the department of natural RESOURCES AND CONSERVATION within 60 days of completing the well. The monitoring well constructor shall retain a copy as a record in his files.
