(1) The case record of each child receiving moderate, campus based or intensive level therapeutic youth group home services must contain the documentation required by this rule in addition to the documentation required by ARM 37.97.524:

(a) referral form/authorization for services;

(b) individual treatment plan, signed by the LCS, which documents the child's response to treatment (progress or lack of progress) , and the staff's interaction and involvement with the client; and

(c) weekly clinical progress notes, reviewed and signed by the LCS, which summarize the child's program participation and psychosocial/behavioral status and functioning.
