(1) The application fee is $50.

(2) The annual renewal fee is $50.

(3) All fees:

(a) must be submitted with the application;

(b) must be paid by check or money order made payable to the Department of Public Health and Human Services; and

(c) are nonrefundable unless the applicant is denied.
